Based in years of experience working with hospitals, health care systems, human service agencies, and other community organizations in Massachusetts and beyond, Â鶹ֱ²¥ now offers technical assistance services to MassHealth Accountable Care Organizations and Community Partners, as a vendor through the Massachusetts Delivery System Reform Incentive Payment (DSRIP) Program.
New models for patient care and cost management present significant challenges and opportunities for organizations to build capacity, refine systems, and coordinate processes to improve patient and population health outcomes. Â鶹ֱ²¥’s deep experience in health care, community health, and social services enables us to facilitate dialogue among these uniquely intertwined systems. We have extensive content knowledge in these areas and a long history of fostering collaboration and building capacity to improve practice and system changes.
Through DSRIP, Â鶹ֱ²¥ offers technical assistance services to Accountable Care Organizations and Community Partners in four domains:
Care Coordination and Integration: Services include assessment, planning, and facilitation services for administrative staff, as well as social service providers and health care teams to define best practices and integrated work flows around behavioral health and health care provision. Subject matter expertise in social determinants of health, behavioral health, and substance use prevention and treatment systems reinforces our understanding of healthcare and social service provider needs in the areas of integrated health systems.
Community Based Care and Social Determinants of Health: Services include facilitation of and consensus-building among multi-stakeholder partnerships, developing and adapting training curricula for target populations, identifying metrics and developing effective referral pathways related to the social determinants of health and other topics, planning for community-based behavioral health innovation models, and coordinating efforts of stakeholders to develop and implement policies related to the integration of health care and public health.
Consumer Engagement: Services include developing and conducting online and in-person trainings on topics such as cultural competency, opioid overdose management and prevention, and partnership-building across multi-sector coalitions, among others; supporting public health and healthcare workers in integrating workflows and building professional capabilities; and leading assessment and planning processes of community health initiatives using a participatory approach that engages stakeholders and residents alike.
Performance Improvement: Services include strategic planning and performance improvement services including executive and leadership coaching, organizational and leadership assessment and planning, leadership and change management and operations planning, and convening of stakeholders and coalition facilitation around shared goals to help healthcare systems strategically address social needs and improve health outcomes within their communities.
